New Student Requirements
All new students are required to submit immunization information. Each item is listed below. Please follow the link to the secure Student Health web portal to record your immunizations, and to upload your form directly into your electronic medical chart.
Failure to Complete
After completing the online requirements, you MUST submit a hard copy of EITHER a signed copy of your immunization form OR an official copy of your immunization records from your health care provider or school. If you received your immunizations in Maryland you can also access your records at the . The Tuberculosis Screening Questionnaire must still be completed online in the secure portal.
You should upload an image of your immunization form directly into the Student Health Web Portal on the immunization page.
All health forms must be completed before July 31, for the fall semester, and January 15, for the spring semester. Failure to complete the requirements by the due dates will result in a $100 immunization late fee and a hold on your account that prevents you from registering for classes the next semester.

Health History
Please complete the health history form located on the secure Student Health web portal.
Please note: newly admitted students will have access to the portal once they receive information that their ÐÇ¿Õ´«Ã½ GullNet account has been converted to an ÐÇ¿Õ´«Ã½ student account.
A physical exam is not required unless you are an athlete. (Please contact Athletics for specific requirements at 410-543-6355)
Insurance Information:
All students are required to submit documentation of their health insurance on the secure Student Health web portal.
Students who are under the age of 18:
To receive a vaccine or any treatment for routine injury or illness at Student Health Services, students under 18 years of age must have consent from a parent or legal guardian.
